Please bring child's current immunization record to the visit. A parent or guardian must provide written consent for children under 18.

Required vaccines :

• Polio
• Diphtheria
• Pertussis (whooping cough)
• Tetanus
• Measles
• Mumps
• Rubella
• Varicella (chickenpox) (see below for further details)
• Hepatitis B (see below for further details)
• UPDATED: Beginning with the 2008-2009 school year, the state of Kansas will REQUIRE:
  
  -- Hepatitis B vaccination - three doses required through fifth grade; and
  --Varicella (chickenpox) vaccine - one dose required through fifth grade.
  
  Children with a history of chickenpox disease need a written statement from a physician or lab confirmation. A parent's verbal history of the disease is also accepted.
