Childhood Immunizations

 

NOTICE: The general immunizations clinic will be closed on 11/24/09, due to the public H1N1 clinic scheduled for that day.

 

 

Selected self-pay vaccine is available for those who don't meet criteria. Click here for pricing information. A parent or guardian must provide written consent for children under 18. Please bring child's current immunization record to the visit.

  • Hepatitis B
  • Rotavirus
  • Dtap
  • Haemophilus influenzae type B
  • Pneumococcal
  • Polio
  • Influenza
  • MMR (Measles, Mumps, Rubella)
  • Varicella (Chickenpox)
  • Hepatitis A
  • Meningococcal

Paying for Services

Please bring proof of income for every wage earner in the household. Valid proof of income includes:

- recent pay stub:

- W-2, or last tax return: or
- employer letter with hours worked and wage per hour.
