School Immunization Requirements

 

NOTICE: The general immunizations clinic will be closed on 11/24/09, due to the public H1N1 clinic scheduled for that day.

 

 

NEW! Tdap vaccine is available for ALL 11-year olds to meet the 2009-2010 school requirements. Cost is $14, offer ends July 31, 2009.

 

Please bring child's current immunization record to the visit. A parent or guardian must provide written consent for children under 18.

 

Required Vaccines:

  • Polio
  • Diphtheria
  • Pertussis (whooping cough)
  • Tetanus
  • Measles
  • Mumps
  • Rubella
  • Varicella (chickenpox) (see below for further details)
  • Hepatitis B (see below for further details)

 

UPDATED: The following vaccines are REQUIRED by the State of Kansas for the 2009-2010 school year for any individual who attends school, a preschool, or a childcare program operated by a school:

  • Diphtheria, Tetanus, Pertussis (DTaP): five doses required. Four doses acceptable if dose four given on or after the fourth birthday.

  • Tdap: booster dose required at 11 years of age.

  • Poliomyelitis (IPV/OPV): four doses required. Three doses acceptable if dose three is give on or after the fourth birthday.

  • Measles, Mumps, Rubella (MMR): two doses required.

  • Hepatitis B: three doses required through grade nine.*

  • Varicella (chickenpox):
    - two doses required for Kindergarten;
    - one dose required for grades one through nine*
    - No vaccine is required if there is a history of varicella disease documented by a licensed physician/lab.
    -Two doses are currently recommended by the Advisory Commitee on Immunization Practices (ACIP) for all ages.

  • Haemophilus influenza type b (Hib): three doses required for children less than five years of age in preschool or a childcare operated by a school.

  • Hepatitis A: two doses required for children less than five years of age in preschool or childcare operated by a school.

  • Pneumococcal (Prevnar): four doses required for children less than five years of age in preschool or childcare operated by a school.

*All students will be required to have three doses of Hepatitis B and two doses of varicella for the 2010-2011 school year.

 

In addition to the immunizations required for school entry lsited above, the 2009 ACIP recommendations also include the following for school children:

  • Menincococcal (MCV4): one dose recommended for children 11 years or older; not required for school entry.

  • Human Papillomavirus (HPV): three doses recommended for females 11 years or older; not required for school entry.

  • Influenza: yearly vaccination recommended for all ages six months of age or older; not required for school entry.

 

 

Paying for Services

Please bring proof of income for every wage earner in the household. Valid proof of income includes:

- recent pay stub:

- W-2, or last tax return: or
- employer letter with hours worked and wage per hour.
