Childhood Immunizations

Clients 18 years and younger with private health insurance are required to provide documentation of immunization coverage.

All immunizations are given on a walk-in basis.

 

Self-pay vaccine is available for those who don't meet criteria for state funded vaccines.

 

A parent or guardian must sign a Johnson County Department of Health and Environment written consent form for children under 18. Please bring child's current immunization record to the visit.

Paying for Services

Please bring proof of income for every wage earner in the household. Valid proof of income includes:

- recent pay stub:

- W-2, or last tax return: or
- employer letter with hours worked and wage per hour.
